Come join our awesome Minnesota Float Team as a Social Worker traveling to our senior care centers in the Minneapolis/St. Paul, Sleepy Eye and Rochester areas in Minnesota. You can work a minimum of 4 shifts per month or full-time hours. It is up to you!

The Minnesota Float Team is part of Volunteers of America National Services (VOANS) which serves as the Housing and Healthcare affiliate of the Volunteers of America parent organization.

About the job:

To support the Director of Social Services in the care and treatment of residents entrusted to the facility. To serve as a liaison and role model to the community for the facility. Represents the Social Services department, other care center staff and the facility as a whole to the community.

Required Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Bachelor's degree in Social Work or related degree in social sciences.
  • Licensed where applicable by the State; State requirements may dictate supervision and degree type.
  • Minimum of one-year of Social Work experience, preferably in a long-term healthcare setting; knowledge of long-term health care and public health settings; familiarity with Federal and State regulations and requirements for long-term care.
  • Ability to read, speak and write fluently in English; excellent communication and phone skills; ability to prepare written assessments and correspondence; ability to read and interpret rules and regulations; carry on negotiations for resident's welfare on the phone.
  • Ability to add, subtract, multiply and divide in all units of measure using whole numbers, common fractions and decimals; ability to comprehend and execute simple statistical functions to create standard graphs and tables.
  • Ability and willingness to work flexible hours and on-call duties, such as during evenings and weekends, for planned activities or special events, family meetings and conferences, admission calls, admissions and tours.
  • Negotiation skills and ability to carry on several levels of reasoning at once during problem solving sessions; ability to think quickly and to write contracts and complex written arguments.
  • Ability to work with residents of varying levels of responsiveness.
  • Familiarity with word processing, data processing and computer entry skills to match existing clinical management software at facility.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.

Essentials:
  • Coordinates resident's admissions.
  • Coordinates and leads transfer and discharge team.
  • Completes assessments of residents as necessary for Care Planning, MDS, RAPS, PPS and other required documents.

The Volunteers of America National Services Minnesota Float Team is a team of experienced professionals that assist in the facilities that we serve, such as the Maplewood Care Center, The Homestead at Maplewood, Elder Care Center (Minnetonka), The Homestead at Anoka, The Homestead at Coon Rapids, Sleepy Eye Area Home Health, The Homestead at Rochester and other senior care centers in the Minneapolis/St. Paul Area.
