About the job:

The Licensed Practical Nurse provides general and skilled nursing care to participants, executes interventions in accordance with the plan of care, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family education and communicates participant's condition to primary caregivers, physician and care team.

• Graduate of a school of professional nursing
• Cardio Pulmonary Resuscitation (CPR) and intravenous (IV) certification required

• Unencumbered license as a Licensed Practical Nurse (LPN) in the State of Colorado required.

• A minimum of one year's experience in working with the frail or elderly population

• Must have a valid driver's license, proof of insurance and have means of transportation

Essentials:

• Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

• Education: Graduate of a school of professional nursing. Cardio Pulmonary Resuscitation (CPR) and intravenous (IV) certification required.

• Licensure: Unencumbered license as a Licensed Practical Nurse (LPN) in the State of Colorado required.

• Experience: A minimum of one year's experience in working with the frail or elderly population.

Skills and Knowledge:

• Ability to read, speak and write fluently in English.
• Knowledge base of physical, mental, and social needs of the frail elderly and their families.
• Skilled in physical assessment and triaging. Ability to react calmly and effectively in emergency situations.
• Skilled in preparing and maintaining medical records.
• Knowledge of nursing standards and best practices.
• Knowledge of medical equipment and instruments.
• Strong interpersonal communication and phone skills. Ability to prepare written assessments and correspondence. Ability to read and interpret rules and regulations.
• Familiarity with word processing, data processing, and computer entry skills to match existing management and other software used by the program.
• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
• Ability to apply creative problem-solving skills to complex issues. Ability to plan, prioritize, and follow-up on responsibilities.
• Ability to foster collaborative working relationships. Ability to supervise, direct, coach, and develop direct reports and larger team. Demonstrated ability to work with a diverse team and flourish in a flexible work environment.
• Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
• Knowledge of infection control practices and quality assurance and improvement.
• Knowledge about the PACE model and philosophy of care.
• Willingness to provide on-call coverage within the nursing rotation.

ESSENTIAL FUNCTIONS:

• Functions as a member of the Care Team. Maintains regular attendance at, and participants in Care Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery. Effectively communicates with team mates during meetings or one-on-one conversations.

• In coordination with the Care Manager and other nursing staff, develops and implements a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care. Maintains aligned, comprehensive plan of care for all participants living in residential facilities and coordinates care between facility and Care/Interdisciplinary Team.

• Conducts nursing evaluations as needed and coordinates with the Care/Interdisciplinary Team to develop a comprehensive plan of care for each participant.

• Performs general and skilled nursing care to participants as assigned. Implements the plan of care and physician orders in coordination with the Care/Interdisciplinary Team. Observes, records, and reports participant's condition and reaction to drugs and treatments to providers. Triages participants to determine priority of care and assists in the treatment, examination, and testing of participants.

• Participates in nursing on-call rotation to provide after-hours and weekend assistance and care, as needed, to PACE participants.

• Receives participant complaints, triages, and responds to participant needs.

• Reviews and signs off orders and disseminates orders and instructions to appropriate staff and contracted personnel. Maintains and reviews participant records, charts, and other pertinent information. Completes documentation and charting in accordance with organizational policy.
