Job Summary

Responsive to provide competent clinical nursing practice across the life-span, facilitates transitions across care settings, and coordinates care delivery with other disciplines for improving patient care quality outcomes as well as decreasing unnecessary utilization and associated costs across patient populations and health care settings. Committed to the daily ambulatory nursing practice performance primarily is composed of focused assessment supporting telephonic patient triage, patient education, direct care interventions, collaborative leadership, care coordination, and clinical and clerical support to providers and team.

Schedule:
Mon - Fri 9a - 5:30p. 1st 90 days he/she will follow preceptor's schedule (e.g. 8a - 4:30p). Potential future shift rotation: 11a- 8:30p (Mon - Wed); 9a - 5:30p (Thurs); Off (Fri); 8a - 12p (Sat). Shift will change according to staffing needs. Note: Evening/Sat temporarily suspended.
