Position Details:
Salary: $39-$41.20 based on experience
Schedule: M-F, occasional weekends

The QAA/QAPI Manager is responsible for facilitating the implementation and maintenance of the QAA/QAPI process. The process provides on-going performance improvement and improved outcomes. The QAA/QAPI Manager works with all stakeholders to integrate this process into the services provided.

The QAA/QAPI Manager serves as the coordinator of all departments working to create a just culture for safe reporting, QAA/QAPI activities, PIP's and process improvements and outcome tracking.

REQUIRED

  • Extensive experience in a health care setting; must have the knowledge and education necessary to understand internal and external customer needs in the health care environment.
  • Bachelor's degree in a health care related field
  • Demonstrates an ability to utilize computer systems and applications.
  • Numerical ability necessary to work with charts and graphs and other tools of QAA/QAPI Coordinator.
  • Demonstrates a strong belief in the importance or quality and acts as a role model for desired quality competencies. Commitment to following concepts of QAA and the QAPI process.
  • Understands group process and group dynamics.
  • Effective oral and written communication skills.
  • Teaching and coaching skills.
  • Ability to work cooperatively and effectively with all levels of staff.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
  • Understanding of current Antibiotic Stewardship, Infection Prevention and Infection Control and OSHA guidelines.
  • Knowledge of state and federal requirements for long term care.
PREFERRED
  • Performance Improvement education/Analytics interpretation is preferred.
  • RN or LPN license in the state of KY
Essential Functions: Individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

1. Develops, and guides the implementation of the annual QAPI plan.
2. Ensures staff integration into the QAPI process through Continuous Quality Improvement education and developing opportunities for input.
3. Analyses of risk management data, and QAPI activities data with the objective of identifying and controlling loss to protect the assets of the organization.
4. Conducts annual satisfaction surveys of the participants and caregivers of Senior CommUnity Care and reports findings.
5. Reports data and information in the manner and at the time intervals specified by CMS and the State agency pertaining to participant care activities and outcomes.
6. Facilitates quality assurance-related communication between contract facilities and SCC Interdisciplinary Team and employees.
7. Facilitates and guides participants through both internal and external appeals process.
8. Facilitates the quality assurance/performance improvement process for various services areas within Senior CommUnity Care.
9. Oversees the process to evaluate, and resolve medical and non-medical grievances by participants, and/or their representatives, ensures information is reviewed for incorporating issues in development of Quality Improvement Initiatives and annual QAPI plan.
10. Attends the Participant Advisory Committee, assists in development of agendas, and acts as meeting facilitator.
11. Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
12. Practices Universal Precautions and follows all appropriate infection control procedures.
13. Participates in continuing education classes and any required staff and training meetings.
14. Performs relates duties as required or requested.
