This position is responsible for maintaining the integrity of the content of electronic health records by performing in-depth investigation of questionable data entry and correcting as appropriate. The Chart Correction Analyst provides issue identification, assessment, resolution, and technical support to achieve desired outcomes and compliance with Shepherd Center policies/procedures and Joint Commission standards and other regulatory agency standards.

JOB RESPONSIBILITIES

  • This role analyzes chart correction scenarios and performs subsequent hands-on technical chart corrections as part of daily work responsibilities.
  • Serves as an organization resource for issues involving chart correction, identity, and instructional support.
  • Works at the center of the chart correction process, facilitating correction of clinical information throughout all inpatient and outpatient information systems.
  • The individual coordinates the parties and interdisciplinary departments involved in each case (i.e., Patient Access, clinical departments, Pharmacy, Radiology, Lab, Physicians) and is responsible for ensuring all necessary corrections are made in a timely manner. Each case is validated prior to closure to ensure that documentation is complete and accurate to support patient safety and continuity of care.
  • Performs quantitative and qualitative analysis of medical records of discharged patients.
  • The EHR is reviewed for missing documents, incomplete information on existing documents, and missing signatures to ensure the record is complete and accurate.
  • Assistance is provided to physicians when they are completing their deficiencies.
Required Minimum Education
  • High school graduate or GED equivalent required .
Required Minimum Certification
  • RHIT certification preferred.
Required Minimum Experience
  • Minimum of 2 years' experience in a HIM environment or healthcare environment required.
  • One year of Medical Record and/or scanning experience within OnBase preferred.
  • Epic Experience preferred.
  • Six months of analysis experience preferred.
PHYSICAL DEMANDS
  • Work is primarily seated but may involve walking or standing briefly. Occasionally lifting or carrying up to 15 pounds
