Primary Responsibility / Summary

Counsels all assigned participants referred for vocational services and at designated intervals. With the participants, develops individual plans to help maximize productivity and develop appropriate workplace behaviors.

Essential Duties & Responsibilities

1. Provides vocational counseling and individualized assistance to all assigned participants.
2. Completes reports for referring agency regarding participant progress in a timely manner. Maintains a complete physical and electronic case record - according to CARF, agency, and other regulatory standards for each participant served including case notes, individual service plans, monthly /quarterly progress reports, incident reports, Adult Protective Services reports, and other relevant/required documentation.
3. With the input of participant, develops individualized plan to address vocational rehabilitation needs based on the individual's strengths, abilities, needs and goals. Makes recommendations as to services the company or another vendor can provide. Coordinates with Intake and Transition Counselor to make recommendations for next step for participant.
4. Through the use of positive behavior modification techniques, hand-over-hand training, performance contracts, and other interventions, ensures that the participant performs job assignments in following steps outlined in task analyses, maximizes productivity, and develops appropriate workplace behaviors.
5. Communicates participant progress to concerned departments and referral sources. Schedules and coordinates case conferences, briefings, other meetings and interventions as needed.
6. Reviews and assesses productivity measurements to ensure accuracy and participant's abilities. If productivity falls below minimum standards for the program, develops strategies to increase productivity or makes referrals to another more appropriate program.
7. Assists with the completion of documents or activities related to, but not limited to: time sheets, daily attendance, monthly statistical reports, and daily production goals.
8. Serves as an advocate for participants, elevates issues to leadership, referring agencies or outside protective services, as required. Completes incident reports and protective service reports.
9. In the rare case of downtime or work slowdowns, develops and provides meaningful training and activities that help to enhance the participant work and social skills.
10. This position may require some travel, a valid driver's license and state-required auto insurance are necessary. Driving records must be acceptable by the company's insurance carrier.
11. Other duties and special projects as assigned.
12. Participates in continuous quality improvement activities.
13.This job has limited supervisory responsibilities for program participants.

Education & Experience

• Minimum 3 years of case management and counseling experience required.
• Bachelor's degree from four-year college or university in Vocational Rehabilitation Counseling, Psychology, Social Work, or a related field required. Master's degree preferred.
• Ability to work with minimal supervision.
• Non-profit experience preferred.
• Experience serving persons with disabilities; ADA knowledge required.
• Bi-lingual fluency (e.g., English/ASL, English/Spanish, etc.) and ability understand another culture as required by the program or local community helpful.
• Proficient in MS Word, Excel, Power Point and Outlook.
• Excellent written and verbal skills; able to write detailed, descriptive case notes and reports.
• Certified Rehabilitation Counselor (CRC) highly desirable.
