NEW HIRE SALARY RANGE: $53,975 - $63,500/year

POSITION SUMMARY: Administer, manage, and coordinate with various Denver Metro inpatient psychiatric hospitals and WellPower teams for individuals who are hospitalized. Manage, oversee, and assist with the communication between various hospital staff, WellPower management, and WellPower adult treatment teams to ensure a smooth transition back to the community for the people we serve who the hospital/s staff deem ready for discharge. Coordinate out of County referrals. Determine eligibility for services and collect referral information for the WellPower Access team. Complete Behavioral Health Assessments to admit individuals to WellPower services.

ESSENTIAL JOB FUNCTIONS:

      • Attend meetings virtually or in person with various Denver Metro inpatient psychiatric hospitals in Denver metro to discuss referrals and people we serve's treatment needs.
      • Manage and coordinate discharge planning between WellPower and various inpatient psychiatric hospitals.
      • Ensure that the relevant hospital takes full responsibility for the final decision to discharge the individuals in their care.
      • Communicate with appropriate managers to keep them informed of problems or issues as they arise to maintain effective and proactive communication with hospitals.
      • Arrange and assign discharge plan responsibilities between all professionals involved and manage the implementation of these responsibilities.
      • Coordinate discharge transitions to WellPower residential housing and manage the assigned responsibilities assigned in the discharge plan.
      • Complete initial intake evaluations.
      • Research individuals' information on multiple computer platforms.
      • Schedule intake appointments in the Electronic Medical Record.
      • Understand multiple insurance coverage for Behavior Health.
      • Coordinate the scheduling of appointments and meetings with WellPower team members and manage the attendance at these meetings.
      • Complete and document phone screening for referrals, complete hospital referral summary, track hospitalizations, and complete progress summaries.
      • Provide Behavior Health Assessment including a provisional diagnosis.
      • Complete 30-day treatment plans with the individuals that we serve.
      • Ability to meet hospital requirements to be in their facility.
      • Other duties as assigned.

EDUCATION: Master's degree in psychology, social work, or other related human services degree.

EXPERIENCE: Three years' experience preferred working with those that have severe and persistent mental illness. Some project coordination, managerial experience preferred.

SKILLS AND COMPETENCIES:
      • Knowledge of the local mental health system and resources.
      • Ability to problem-solve with others.
      • Excellent organizational and prioritizing skills.
      • Knowledge of evidence-based practices.
      • Ability to assess crisis/emerging situations and intervene appropriately.
      • Effective written and verbal communication skills.
      • Familiar with DSM-5.
      • Assist in 72 hour holds when needed and able.
      • Ability to own and operate an automobile.
      • Ability to welcome and celebrate individual's culture from an inclusive and equity lens.

SUPERVISORY RESPONSIBILITIES: No formal supervisory duties. Work closely with management staff to create and maintain cooperative relationships and effective communication between WellPower and hospitals.

MACHINES AND EQUIPMENT TO BE USED: Computers, calculators, fax machines, copiers, telephone, and a variety of other office/clerical equipment. Ability to own and operate an automobile.

TYPICAL PHYSICAL DEMANDS: Requires sitting, standing, bending, and reaching. May require lifting to 50 pounds. Requires manual dexterity sufficient to operate standard office machines such as computers, fax machines, the telephone and other office and/or clinical equipment.

WORKING CONDITIONS: May require occasional evening or weekend hours.
