Description

The West Virginia University Research Corporation is seeking applications for a Patient Navigator with the Center for Excellence in Disabilities located in Big Chimney, WV.    The purpose of this position is to provide care coordination, referral, and follow up services to patients of the Partners in Health system who are enrolled in a research study to receive additional care coordination services. 

This position will follow-up on results from a patient assessment of social service needs in the following areas: housing, transportation, food insecurity, utilities, and interpersonal violence. Enrolled patients who have identified needs in one or more of these areas will receive coordination services to link them to providers of those services in their region. This position will interact with patients from a variety of clinical and medical facilities within the Partners in Health and West Virginia University system. Facilitate patient connection with provider of needed service and will follow up with the patient after initial referral is made to see whether services were received, what challenges were experienced, and how satisfying the process was to the patient.  

In order to be successful in this position, the ideal candidate will: 

  • Function as a screener, which will require going to designated clinical delivery sites and conducting social determinants of health screens on patients who meet the qualifications.
  • Serve as an internal linkage to health care teams and enrolled patients by attempting to directly connect the patient to an established social service provider and follow up on any barriers for the patient to access the full scope of services available for that area of need. 
  • Once a social service need is identified by a screening tool, the Navigator works to schedule and relay information between patients and the social service provider. 
  • Track any information the provider may need to provide needed services and patient appointment with provider as needed.
  • Communicate patient needs to provider as needed, provide support to patient to establish communication with provider for ongoing needs.
  • Confirm services to both provider and patient.
  • Follow up with patient at established program intervals to see if services are received, obtain patient satisfaction, and identify ongoing challenges.
  • Maintain sensitive, private health information in accordance to HIPAA standards and regulations. 
     


Qualifications

  • Bachelor's degree in an allied health field or a related discipline;
  • Six (6) months of experience in a medical, clinical, or social services (e.g., housing office, transportation provider, interpersonal violence group) setting.
  • Or an equivalent combination of education and/or experience.
  • Ability to advise clients or community groups on social service issues and providers.
  • Ability to assess individual social service needs.
  • Knowledge of principles and processes for providing personal services. This includes client needs assessment, meeting quality standards for services and evaluation of client satisfaction.
  • Knowledge of administrative and clerical procedures and systems such as word processing, managing files and records, designing forms, and other office procedures and terminology for required record keeping in project.
  • Skilled in complex problem solving, including identifying complex problems and reviewing related information to develop and evaluate options and implement solutions.
  • Ability to establish and maintain interpersonal relationships, including developing constructive and cooperative working relationships with others, and maintaining them over time.
