The Intensive Transition Services (ITS) program offers support to adults who have both a serious mental illness and a developmental disability in a multidisciplinary team setting. Participants receive clinical therapeutic interventions, care coordination and help dealing with other issues in their daily lives.

The ideal candidate will possess the following qualifications:
• Masters degrees in Social Work, Psychology, or another human service related field.
• Current and unrestricted California license with the Board of Behavioral Sciences such as Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT)or Licensed Professional Clinical Counselor (LPCC)
• Three to five years of experience in the behavioral health field working with individuals with developmental disabilities.
• At least one year of experience of case coordination and case management services.
