Description:
The Judge Group needs Home Care RN Case Managers in Montgomery, Bucks, Delaware, Chester or Philadelphia counties in Pennsylvania!
Overview:
- Manages, oversees, and provides primary patient care delivery to a select group of patients supporting the patient centered care model, assuring quality and maintaining open communication.
- Presents the patient at interdisciplinary team meetings as scheduled and as needed.
- Collaborates with team members to assure patient care delivery needs are met.
- Interacts with all members of the care group both internal and external to the organization as needed.
Qualifications:
- Current PA RN License
- Valid PA Drivers license
- At least 1 year home health or acute care experience
- BSN required **unless they have current home care experience**
- CPR certification
